We encourage all employees, contractors, and stakeholders to use this service when discretion and anonymity are critical. For matters not requiring anonymity, please adhere to standard reporting protocols as outlined in company policies.

 

What to Report

You can report a wide range of concerns, including but not limited to:

  •    Fraud, Theft, or Embezzlement

  •    Violation of Company Policies

  •    Misuse of Resources or Property

  •    Vandalism, Sabotage, or Threats


This service also covers improper behavior, falsification of documents, and violations of local or federal laws. All reports are reviewed and handled according to the company's investigation protocols.


Confidentiality and Anonymity

Rios Health Plan maintains a confidential and anonymous compliance reporting system. You may report any compliance concern anonymously through the Compliance Phone Line, Fax Line, or Email listed. You are not required to provide your name or identifying information. All reports are handled confidentially and without retaliation.
